Form Ortho Co. is an orthodontic practice operating in Ontario, Canada. The Services are provided for general information, administrative convenience, and patient communication.

Not Medical Advice / No Doctor–Patient Relationship Through the Website

Content on the website is provided for general informational purposes only and is not a substitute for professional dental, orthodontic, medical, or healthcare advice, diagnosis, or

treatment. Using the website, sending messages through online forms, or receiving automated communications does not create a doctor–patient relationship. A doctor–patient

relationship is established only through an in-office (or otherwise formally established)

clinical relationship with appropriate documentation.

No Emergency Use

Do not use the website, email, or text messaging for urgent or emergency matters. If you

believe you are experiencing an emergency, call 911 or go to the nearest emergency

department. For urgent orthodontic issues, please call our office directly during business

hours.

Appointments and Online Requests

You may be able to request appointments or submit inquiries through our website.

Submission of a request does not guarantee an appointment. Appointments are confirmed

only when you receive confirmation from Form Ortho Co. We may contact you by phone,

email, or text message (if you have opted in) to confirm scheduling or obtain additional
